Student Accident Insurance
CA K-12 Student Accident Insurance Program
Information
TUSD carries an excess student accident insurance policy, which insures students when medical claims are incurred as a result of a covered accident or injury. This policy provides up to $25,000 of coverage for students for any accidents related to school activities (i.e., athletics; on-campus, during any school-sponsored activities; off campus, during any school-sponsored activities; and to and from school). This can help both insured families with deductibles and those without insurance cover medical costs from an accident.
For specific coverage details, please refer to the CSAIP K-12 Student Accident Insurance Program Brochure.
Claims Procedures
Always keep a copy of all claim-related documents. Written proof of loss must be submitted within 90 days from the date of such loss.
- Contact your school to obtain an accident claim form. Schools will complete Part 1A of the BMI Benefits Accident/Injury Claim Form.
- Complete the claim form in its entirety and submit it to BMI Benefits within 90 days from the date of the accident.
- Submit all itemized bills and primary insurance E.O.B.s to BMI Benefits for processing outstanding balances due to a Covered Accident. You may also contact the medical provider and submit BMI's information as your secondary coverage, which would allow the provider to bill BMI Benefits directly with the necessary billing documents.
